Every client will review and receive a copy of United Summit Center's Client rights brochure which described in detail the various rights each clients has as a consumer of United Summit Center. These rights include: treatment services under conditions that support the client's liberty, an individualized treatment plan or strategy, ongoing participation in the planning of services, the right to refuse treatment, a humane treatment environment, the confidentiality of medical records, the right to assert grievances, access to a qualified advocate, availability to services without discrimination, to exercise civil rights and to be referred as appropriate.

YOUR RIGHTS UNDER TITLE VI
United Summit Center operates its programs and services without regard to race, color, or national origin in accordance with Title VI of the 1964 Civil Rights Act. Any person who believes that she or he has been aggrieved by any unlawful discriminatory practice under Title VI may file a complaint with our agency.
